Important Information about how to fill this form.

For single Applicant:

If you are a single applicant in the application then just fill out the form normally and give details wherever you select "YES"

For Multiple Applicants:

If there are multiple applicants in this application. For example, you have accompanied members included in your application then four rules apply here.

All your responses will be considered "true" for all the applicants in your application. Meaning if you select "NO" in any or all questions. That means you are saying "NO" for every applicant included in this application.
If the answer is "YES" for all or any questions below, then you would have to provide the Name of the applicant on whom the "YES" applies and complete details about the issue.
If there are more than one applicants for which you select "YES" for any questions below then you must name each applicant's name and his/her associated details about the issue.
If the "YES" matter relates to a criminal conviction, provide:
- the date and nature of the offence
- full details of the sentence
- dates of any period of imprisonment or other detention.

Note: It's better to discuss your issue with your Migration agent instead of hiding it. Department uses a data matching service that may detect any information that you may choose to hide. Pay special attention to the traffic offenses, ticket and fines in Australia.
